Improving Diabetes Care: Focus on Solo and Small Practice

The Diabetes Quality Improvement Project is proud offer a new publication for physicians called Improving Diabetes Care: Focus on Solo/Small Group Practices. This publication aims to help physicians better identify the latest evidence based guidelines for diabetes care and management through the perspective of four physicians and their stories of practice quality improvement. The physicians highlighted have struggled with limited resources, limited staffing and complex patient populations yet they have been able to adopt evidence based practices to improve the quality of
care in their respective practices. A central theme among all 4 practices is team care; the physicians have been successful in improving care in large part because they have utilized and coordinated their medical assistants and medical office to staff to a high level of professionalism in diabetes care
management. This resource will help physicians approach practice improvement by making existing resources more efficient and maximize their potential value.

Diabetes and Cardiovascular Disease Provider Reference Guide, 2nd Edition 2011

The California Medical Association (CMA) Foundation’s Diabetes Quality Improvement Project is pleased to announce the improved and updated 2nd edition of the Diabetes and Cardiovascular Disease Provider Reference Guide (PRG), 2011. This reference guide was developed with the goal of bringing healthcare providers in a variety of practice settings current clinical guidelines, evidenced based best practices and patient self management resources, to assess, manage and address diabetes as cardiovascular disease complication. The guide was coordinated by the California Medical Association (CMA) Foundation with the help of 29 physicians and healthcare professionals who contributed to the update of the content.

Diabetes Care Guidelines/Flow Sheet

The CMA Foundation's Diabetes Quality Improvement Project has developed a flow sheet that incorporates the diabetes care guidelines approved by the American Diabetes Association, Standards of Medical Care in Diabetes, 2011 and the American Association of Clinical Endocrinologists, Medical Guidelines for Clinical Practice for Developing a Diabetes Mellitus Comprehensive Care Plan.
This resource was developed with the assistance of the several frontline health care providers as an effort to improve the way in which patients with diabetes receive quality care.

Diabetes and Your Nerves

In collaboration with the UCSF Neuropathy Center, the CMA Foundation has completed a patient education handout for patients who have Diabetic Peripheral Neuropathy (DPN) called “Diabetes and Your Nerves”. The handout is available in both English and Spanish.
DPN is the most common polyneuropathy in the United States and worldwide and contributes to incidences of lower limb amputation due to patients’ loss of sensation and associated infections. The handout aims to educate patients so that they can recognize symptoms of DPN and other forms of neuropathy, seek professional help, and take action to prevent further nerve damage.
