The California Medical Association Foundation and NEPO are teaming up with the American Medical Association to raise awareness and educate physicians and health care providers about tools and resources available to help reduce the incidence of type 2 diabetes within the communities they serve. In partnership with the Centers for Disease Control and Prevention (CDC), the AMA has created a special online toolkit to help physicians and care teams: • Identify and refer at-risk patients to a CDC-recognized diabetes prevention lifestyle change program within their community with our online toolkit (Visit thecmafoundation.org/Programs/Prediabetes-Diabetes to learn more.) • Determine potential medical cost savings from referring high-risk patients to a CDC-recognized diabetes prevention lifestyle change program using the diabetes prevention cost-savings calculator • Strengthen population health efforts through educational modules that include performance improvement CME and CME for training clinical staff on prediabetes • Increase patient awareness of prediabetes with a one-minute risk test If you have questions, email us at firstname.lastname@example.org for answers and information.
